Basic Obedience Group Class Registration Form
To register for the obedience class, please complete the following form. Please be sure to answer all questions and read the waiver form before clicking the "Submit" button. Once completed, this page will direct you further to the Pay Pal payment page as well as the class information page. Please note that you will be officially registered for class once you have submitted your payment of $135 for the 5 week series of classes.

Basic Obedience Training Waiver and Release from Liability Form
1. Voluntary Participation
I acknowledge that I have voluntarily applied to The Barking Bunch for Basic Obedience Group Training. If my application is accepted I agree that The Barking Bunch may drop me from the training program at any time, with or without cause, and in such event only the unearned portion of the training fee will be returned to me. I agree that the success of this training program is contingent on my participation and practice with my dog within and outside of class.
2. Assumption of Risk
I am aware that dog training may be a hazardous activity. I specifically acknowledge that I recognize the risk of taking group class with other people and dogs. This risk is entered into with me being 100% responsible for my dog's behavior at all times in the class environment. I hereby agree to accept any and all risks of injury to myself, my dog or any dogs under my care and custody.
3. Liability & Release
I, as the legal owner/agent of the above mentioned dog have carefully read and fully understand this agreement and do hereby waive and release The Barking Bunch and Nadia Brunner and all officers, employees, lessors and agents from any and all liability of any nature. This includes any injury, death, sickness or damage my dog may suffer during and after the training program. I also agree to indemnify and hold harmless The Barking Bunch and Nadia Brunner and all officers, employees, lessors and agents from any and all claims due to the damage my dog may cause to any family members or any third parties during or after training.
4. Termination of Training
As the owner of the dog I may have the animal withdrawn from training class at any time. However once training has begun, all fees are non-refundable.
5. Miscellaneous
As the owner of the dog I will be responsible for purchasing all necessary equipment the trainer recommends for training my dog.
I understand that if I miss any classes I will be fully responsible for the cost of the classes and will contact the trainer to discuss making up the missed portion of class. Additional fees may apply.
In the event that either party deems it necessary to employ legal counsel to protect their rights under this agreement, the prevailing party agrees to pay all expenses including, but not limited to costs and attorney's fees. This Agreement and Release will be governed by, and constructed in accordance with California law.
All rights to televise, video tape, audio tape, photograph or otherwise promote Basic Obedience Class are vested in The Barking Bunch and it's liaisons. I forever grant The Barking Bunch the right to promote the dog training class and use my, my dog, or any other dog under my care in any manner without further release, consent or compensation.
The training agreement waiver form represents the full and only agreement between the parties. The terms and conditions of this agreement cannot be modified and changed in any way.
BY CHECKING THE BOX AT THE END OF THE CLASS REGISTRATION FORM I AGREE THAT I HAVE CAREFULLY READ THIS RELEASE AND FULLY UNDERSTAND IT'S CONTENT. I AM AWARE THAT THIS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE BARKING BUNCH AND NADIA BRUNNER
